First step mandatory: complaint to healthcare provider itself — complaints officer or disputes committee (Wkkgz-required). Only then IGJ. Template content: your identification, healthcare provider + location, incident chronology, GDPR aspect (which articles breached), prior complaint resolution, evidence (medical record extract, correspondence), requested IGJ action (investigation, sanction, oversight measures). How to file? Online via igj.nl reporting form or call 088-120 50 00. Anonymous complaint possible but less effective. IGJ action: acknowledgement within 5 days, investigation can take months. Measures: warning, order, cease-and-desist, referral to Public Prosecutor. Combination with AP: GDPR aspect parallel to AP. IGJ + AP often share info. On serious medical care error: also disciplinary complaint at Regional Disciplinary Tribunal (RTG). Damages claim Art. 82 GDPR: on medical-data leak typically €1,000-€5,000.
