Newsletter consent form — template (double opt-in)

Double opt-in flow, GDPR + Dutch Telecoms Act 11.7 compliant. Template form + confirmation email + required unsubscribe text.

Last reviewed: 25 May 2026
Newsletter consent = explicit consent for email marketing. GDPR Art. 6(1)(a) + Art. 7 + Dutch Telecoms Act 11.7. Best-practice double opt-in: (1) Form: email + name (optional) + checkbox "I consent to the newsletter from [business]. I know I can unsubscribe at any moment via the link in every email. Privacy statement: [link]." NOT pre-checked. (2) Confirmation email: "Click to confirm your signup" (24-hour link). Only after click = consent valid + add to list. (3) Signup confirmation: "Welcome! You're on the list. Unsubscribe: [link]." (4) Every newsletter: unsubscribe link at bottom + recognisable sender + "why am I getting this?" mini-text. What is NOT allowed: single opt-in (only form, no confirmation) — doesn't prove the email-address owner gave consent. Signup via "tick here if you DON'T want newsletter" (opt-out — prohibited). Signup for other purposes + newsletter in 1 tick box (linked consent). Tools doing this well: Mailchimp, Brevo, ActiveCampaign — built-in double opt-in. Avoid tools supporting only single opt-in. On withdrawing consent: unsubscribe link must work in 1 click — no retention pop-ups, no "are you sure". Fine risk: multiple AP fines 2024 for wrongly set up consent flows in retail + healthcare.
