Photo consent form — template (school photo, event, marketing)

Photo consent is GDPR Art. 7 consent + portrait-right licence together. Mandatory elements: (1) Party identification: you (photographer/organisation) + subject + if minor under 16 parent/guardian (UAVG Art. 5). (2) Specific use per channel — website ≠ social ≠ yearbook ≠ marketing. Separate tick per medium. (3) Duration: standard 5 years + revoke option. (4) Purpose: education, marketing, portfolio — specific. (5) Compensation if commercial — in amateur context explicitly "no compensation requested". (6) Revoke procedure: how + within which period (typically 30 days). (7) Privacy statement link: reference to business privacy. (8) Date + signature. For children <16: parental signature + separate child signature above 12 as "extra approval" (good practice, not legally required). For commercial shoot: model release contract with more detail. On refusal or revoke: within 30 days remove — both digital and physical copies (yearbooks require destruction demand).
